PII detection is best-effort and covers structured PII only (SSNs, cards, emails, phones, IPs — not names, addresses, or free-text). Defense-in-depth for PHI workloads — not a HIPAA control or a substitute for a BAA.

Cache hits are free. Identical deterministic requests (temperature=0) served from cache don't cost a thing — the second eval run is on us.
